9 Week Program: Application Process

  1. Download and print the Application Package. Click on one of the following two format options:
    a. Download in Adobe Acrobat PDF file format.
    b. Download in Microsoft Word format (preferred).
  2. Read the instructions and terms and conditions carefully and complete the form.
  3. Complete the application package, which includes:
    • A completed application form.
    • Typewritten responses (essay style) to questions included in the application form.
    • Photocopy of the page of your passport that contains your photograph and personal data.
    • Copy of your resumé or C.V. (curriculum vitae).
    • Two references (name, phone number and email address if available)
    • Two passport-sized photographs with your passport number written on the reverse side.
    • A U.S. $50 non-refundable application fee (click here for payment methods).
  4. Make a photocopy of all of the above for your records (including a photocopy of your $50 check if paying by check)
  5. Mail the original version (with signature) of the completed application package to (DO NOT staple or fold copies sent to us):
